What to Bring to Your Appointment

  • Your insurance card
  • Physician referral forms if required by insurance
  • A list of current prescription or over-the-counter medications you are taking, including dose and frequency
  • Pertinent information about your medical and surgical history, including any pertinent x-rays or records you may have

Payment and Insurance

Tri-County Urologists is a provider for Medicare and most insurance plans. We provide insurance billing. Anything not covered by insurance will be your responsibility.

We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.

In accordance with the Privacy Regulations created as a result of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), Tri-County Urologists is dedicated to maintaining the privacy of your protected health information (PHI). In conducting our business, we will create records regarding you and the treatment and services we provide to you. We are required by law to maintain the confidentiality of health information that identifies you. We will not use or disclose your PHI without your authorization except as described in our Notice of Privacy Practices, which describes how health information about you may be used and disclosed and how you can get access to your protected health information. This notice is available at all of our offices or it can be sent to you upon request.
